How client can change name in account?

If the changes are minor, below is the process.

The following self-attested documents are required:
  • Minor Name Change Form

  • KRA Form

  • PAN Card copy

  • Aadhaar Card copy
    Additionally, these documents must be stamped with the RM IPV (In-Person Verification) stamp.

Please note, a minor name change refers to small modifications such as adding or expanding initials in a name. For example, changing “A. Kumar” to “Arun Kumar.”  OR The change where pronunciation is not getting changed.

Please follow the below process if there are major changes in name.

To change name in client account, please send a hard copy of Name change form along with supporting document which is client self attested and RM IPV stamp (In Person Verification).

Kindly provide valid government ID Proof having the correct name i.e..
1.Self attested PAN card copy.
2.Publication of name change in official gazette.
3.Self attested KRA form.

If the name changes due to marriage the following documents need to be submitted..
1.Self attested Change of Name Form.
2.Self attested PAN card copy.
3.Marriage certificate or copy of passport showing husband s name or publication of name change in official gazette.
4.Self attested KRA form is required.

Note that if a client resides in Karnataka, please provide below documents 1)Self attested Change of Name Form 2) IPV & self attested pan card 3) KRA form require 4) Notarized affidavit require 5) local & English news paper require with mentioned new name add information.
