The Successor can transfer the securities to an account opened in His / Her name only. Kindly send the following documents:
1) Account Closure Form duly sign by Successor. Format attached here
2) Transmission Form duly sign by Successor (Part I and Part II should be filled by Successor) . Format attached here
3) Original OR Copy of the death certificate of the deceased holder (s) duly notarized.
4) Letter of Indemnity executed by the Successor (s) on nonjudicial stamp paper of Rs 500/-, duly notarized . Format attached here
5) An Affidavit from the Successor executed on non-judicial stamp paper of Rs 100/- duly notarized. Format attached here
6) No Objection Statement(s) from all legal heir(s) (Format as per Transmission Request Form [Part II]).
7) Photo Id proof and Address Proof of Successor duly self-attested.
8) Successor shall be sole holder in demat account.
9) Client Master Report of the Successor demat account duly dp stamp and sign.
10) Declaration Letter 1. Format attached here
11) Require cancelled cheque with IFSC code and pre-printed name of the claimant / Nominee for fund payout.
12) Annexure for Details of Securities. Format attaced here
Note 1 - In case name mismatched of Death holder (in Demat Records and Death Certificate) or /Nominee/Joint holder/ claimant then provide the Affidavit on Rs.100/- stamp paper for name confirmation with mentioned all mismatched names.
Note 2 - If Claimant has provided the Digital Death Certificate copy then RM/Branch has to verify from the concern website and attach the print Screen.
